As a Home First Assessment Nurse, you’ll have the opportunity to get to know your clients one-on-one, helping them live a healthy, happy and safe life at home. 

 

As the Home Care Assessment Nurse at Givens Home First, you will play a pivotal role in enhancing the client care experience through assessments, care coordination, and effective communication with clients and their families. You’ll conduct initial assessments, 90-day reassessments, and supervisory visits, ensuring high standards of care for each client. This role requires a blend of clinical expertise, time management skills, and compassionate communication. You will travel between our two Life Plan Communities, Givens Estates and Givens Highland Farms, to support our Home Care clients.

 

What you'll do:

  • Complete initial client assessments, 90-day reassessments, and supervisory visits to develop individualized care plans that prioritize clients' health, safety, and well-being in the home environment.
  • Create, maintain, and adjust care plans tailored to each client’s needs, updating plans based on assessments, and actively involving clients and families in care decisions.
  • Work closely with the care team, providing input and feedback to ensure the effective implementation of care plans and continuous improvement in client care.
  • Act as a liaison with clients, families, and caregivers, ensuring a clear understanding of care plans, updates, and the benefits of the Home First program.
  • Advocate for the Home First program by demonstrating and explaining the value of the services during client interactions, creating trust, and highlighting the program's impact on quality of life.
  • Utilize WellSky software to document assessments and care plans accurately and promptly, ensuring compliance with all state and federal regulations, including safety and infection control measures.
  • Support adherence to all applicable standards and regulations, including infection control, client rights, and safety protocols.
  • Conduct regular supervisory visits to evaluate caregiver performance, client satisfaction, and the effectiveness of care plans, providing feedback and training as needed.
  • Provide on-call support for caregivers and clients, as part of a rotating schedule, to address any immediate care needs or emergencies outside of regular hours.
  • Oversee and promptly report any observed or suspected cases of abuse, neglect, or exploitation in accordance with legal and ethical guidelines.
  • Contribute to the creation and refinement of policies and procedures related to client assessments and care, aligning them with best practices and ensuring consistency across the team.
  • Practice and enforce safety and infection control protocols, providing a safe environment for clients and caregivers.
  • Stay informed of best practices in home care, participate in training sessions, and support quality improvement initiatives to elevate care standards.

What you'll need:

  • Current RN license in North Carolina.
  • Experience in home care or home health with a focus on geriatric care.
  • Proficiency in WellSky or similar software, and comfort with electronic health records.
  • Active driver’s license and reliable transportation for travel to client homes (mileage reimbursement available).
  • Strong work ethic, including regular attendance, punctuality, and a proactive attitude. 
  • Must be able to work flexible hours to meet the needs of daily operations and adapt to changes in client care or program requirements. 
  • Meet physical demands of the position. Including; the ability to lift 100lbs, move throughout the workday, and stand for long periods of time.

Compensation: $32.00-$36.00 per hour based on experience as an RN, plus $5,000 sign-on bonus, and shift differential pay!
