As Clinical Social Worker, Lead in Virginia, you will be key to our efforts to provide better care and support to vulnerable populations. Reporting to Waymark's Head of Clinical, you will work with vulnerable populations, providing behavioral healthcare services using the Collaborative Care Model in conjunction with patients' primary care providers, supervising other social workers as they provide behavioral healthcare services, and working closely with community healthcare workers, pharmacists, and care coordinators across the state of Virginia.
Develop and test (through on-the-ground pilot programs) protocols and processes for therapists assisting primary care providers, community health workers, pharmacists and care coordinators to improve care outcomes for patients receiving Medicaid
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate
Work closely with a mobile community healthcare worker team
Hire, mentor and supervise junior therapists to act as behavioral health care providers and coordinators
Communicate with patients and providers through multi-modal CRMs including multiple EHRs, text, phone, and video
Participate in QI activities to assure quality, cost-effectiveness, and ROI
Licensed Mental Health Counselor/Professional Counselor or Licensed Marriage and Family Therapist or Licensed Social Worker with good standing in Virginia.
6+ years of experience in behavioral healthcare, including delivery of Collaborative Care Model among vulnerable populations
Experience in multi-disciplinary care management
Positive references from prior mentees and supervisors
Collaborative Care Model certification and experience
Experience providing evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated
LEAN Six Sigma/QI experience, with Green Belt or above Lean certification
Strong analytic skills and ability to work with multiple data registries for proactive patient outreach
