Your north star: Provide and facilitate evidence-based, patient-centered group and individual care to our patients
In this role, you will report to Director of Advanced Practice Providers and have the following responsibilities:
- Provide group care in the midwifery model, as well as individual patient care, during pregnancy, birth, and postpartum across a panel of patients in a virtual setting
- Facilitate virtual sessions of preconception, pregnancy, and postpartum group cohorts per week, including weekday, weeknight (i.e., after 6pm EST), and some weekend sessions
- Meet with group care patients individually for medical visits as needed (prenatal, birth planning, postpartum and problem visits).
- Collaborate with other team members (e.g., physicians, PAs, nurses, perinatal mental health specialists, dietitians, lactation consultants, etc.) to provide whole-person care
- Support clinical team with clinical care consultation and didactic meetings
- Participate actively in population health protocols to help ensure optimal care for all Pomelo patients
- Communicate effectively with patients during and between appointments to ensure questions are answered and concerns are addressed in a timely manner
- Represent Pomelo as a champion for the collaborative care model in the virtual care setting
- Work collaboratively with Clinical Operations to establish processes to continue to standardize care and to roll out new services
Who you are
- Certified Nurse Midwife with a current, unrestricted license in a compact state
- Have 5+ years experience in a hospital and/or healthcare practice, providing clinical care to the maternal-child health population
- Have experience facilitating preconception and/or pregnancy/postpartum groups, in-person or virtually
- Have a sense of urgency about achieving optimal health outcomes for individual patients and the Pomelo population
- Have a growth mindset and are comfortable with continuous improvement cycles, including using provider-level data to improve care
- Enthusiastic about developing patient-centered collaborative care models, leveraging individual and group care
- Willing to work Eastern Time hours and facilitate groups on weekdays, weeknights (i.e., after 6pm EST), and some weekends
- Are internet-connected and able to work remotely
- Passionate about tackling birth inequity and the role structural racism plays in maternal morbidity and mortality

Our salary ranges are based on paying competitively for our company’s size and industry, and are one part of the total compensation package that also includes equity, benefits, and other opportunities at Pomelo Care. In accordance with New York City, Colorado, California, and other applicable laws, Pomelo Care is required to provide a reasonable estimate of the compensation range for this role. Individual pay decisions are ultimately based on a number of factors, including qualifications for the role, experience level, skillset, geography, and balancing internal equity. A reasonable estimate of the current salary range is $110,000 to $135,000. We expect most candidates to fall in the middle of the range. We also believe that your personal needs and preferences should be taken into consideration, so we allow some choice between equity and cash.
