We are Imagine Pediatrics, a devoted and compassionate, multi-disciplinary pediatrician-led medical group that is creating a world where every child with complex medical conditions gets the care and support they deserve. Energized by our mission to reimagine pediatric health care, we are pioneering virtual-first care services that include care coordination and around-the-clock access to complex-care clinicians from home. We don’t replace a child's existing care team. Instead, we collaborate to deliver continuous support to kids, so they can spend less time in the hospital and more time thriving at home and in their communities.

What You’ll Do
As a Resource Specialist at Imagine Pediatrics, you will provide support to patients and families by collaborating with community partners and care team members to close patient non-medical drivers of health gaps. The holder of this position assists in building community relationships and identifying resources, providing support, care, guidance, and education to our complex pediatrics patients and their families through peer support partnerships. Responsibilities include but are not limited to:
- Identifying resources and community supports available to the family
- Provide information, referrals, and assistance with linkage to medical, psychosocial, and basic needs services
- Researches, vets, and ensures appropriate closures of external referrals for families
- Assist with accessing specific programs and supports to address psycho-social needs both within Imagine Pediatrics and in the larger community
- Advocate for and on behalf of clients and families to decrease service gaps and remove barriers to services
- Connect with relevant internal staff and providers to assist with system navigation, including Eligibility, Pediatrics, Behavioral Health, Public Health, etc.
- Collaborate with community-based agencies to support linkage to social supports within the larger community
- Assists in development of an internal community resource library
- Provides linkage to community-based resources and school-based services
- Networks with new and established community resources to increase knowledge of services
- Create, develop, and complete memorandums of understanding (MOU) for partnerships with outside community agencies
- Develop effective working relationships with interdisciplinary care teams
- Participate in ongoing scheduled consultations with an interdisciplinary team
- Hybrid role after 90 day hire period with 10 % in the field work.
- Performs other duties and assumes other responsibilities as assigned by manager

- Must be able to travel between work locations and community-based agencies as needed
- Performs other duties as assigned.
- Bachelor’s degree or higher in healthcare, behavioral health, or social services related field preferred
- Certification as a Community Health Worker highly desired
- Familiar with navigating social service agencies
- Must have reliable transportation, a valid TXDL, and ability to produce a copy of current auto insurance certificate
- Willing to travel to community agencies
- Experience with resourcing and telephonic services
- Knowledge of outlook, Microsoft office, and ability to work in online platforms
- Bilingual Spanish preferred
- Experience collaborating with interdisciplinary teams
