We operate four WelbeHealth PACE (Program of All Inclusive Care for the Elderly) programs in Central and Southern California. The WelbeHealth PACE program provides comprehensive primary care and social services for frail seniors to keep them living safely in the community. In addition, we serve as our patients’ health insurer to meet all of their healthcare needs. As an integrated delivery system with small patient panels, PACE Physicians have ample time to engage with patients. The WelbeHealth PACE Physician will report to the Medical Director and, in collaboration with an interdisciplinary team, will provide assessment, planning, primary care, and continuous medical coverage to PACE participants.

 

What you'll do:

  • Provide primary care and acute illness evaluation and treatment for participants in collaboration with WelbeHealth PACE Physicians and Community-Based WelbeHealth PACE Physicians
  • Perform in-person comprehensive assessments (initial, semi-annual and unscheduled) on participants and coordinate with the Interdisciplinary Team (IDT) to develop a comprehensive care plan for each participant
  • Refer participants to medical specialists as indicated
  • As an active member of IDT, work with Nurse Practitioners, Community-based WelbeHealth PACE Physicians and other members of the IDT to ensure effective care coordination, communication of participant changes and collaboration of care planning decisions
  • Participate in rotating night and weekend call
  • Review outcomes and utilization data, including review of necessity of hospitalizations and use of specialists; establish best practices, and collaborate with clinical staff, WelbeHealth PACE Physicians and IDT to make improvements that reduce unnecessary utilization and improve participant health

Your qualifications:

  • Strong preference for those who have completed 2-step COVID-19 vaccination series or are willing to do so in first 30 days after hire as condition of continued employment
  • Creative and enthusiastic commitment to the “triple aim” – dedication to improving experience, health, and value for PACE participants
  • Ability to build strong working relationships with other providers and care team
  • Excellent clinical, organizational, and communication skills in settings with frail, elderly participants, their families, and the IDT
  • Minimum of 2 years working with the frail or elderly
  • Current knowledge of chronic care/geriatric issues and best practices
  • Graduate of an accredited school of nursing with an RN State License
  • Certification as a Nurse Practitioner with prescriptive authority
  • Working knowledge of WelbeHealth PACE regulations a plus
