Sr. Manager, Clinical Care
What You’ll Do
Sr. Manager, Clinical Care is responsible for leading a team to ensure patient care is delivered at the highest standard. Collaborating with other market people leaders, the Sr. Manager, Clinical Care will establish market priorities and ensure an interdisciplinary approach for all patients. This role is responsible for overseeing the performance of individual clinicians by monitoring the provision of high-quality care and ensuring outstanding patient experiences. This function will oversee Registered Nurse Care Managers (RNCM) and Licensed Clinical Social Workers (LCSW). This role will report to the Director, Regional Clinical Care.
The Day to Day
- Ensures clinical personnel within the region follow all policies and procedures.
- Closely collaborates with Market Operations Directors, Implementation Teams, Clinical Education, Quality, and Regulatory departments to support required training, consistent skills development, and process improvement initiatives for clinicians across the region.
- Ensures employee competency through the development, retention, and performance management of the clinical team. Leads the interviewing, hiring, and evaluation process of clinical personnel within the region.
- Manages the clinical team with direct management of RNCMs and general oversight of the performance of the greater market team.
- Accountable for managing and supporting clinical team daily productivity and removal of associated barriers to ensure achieving expectations.
- Fosters positive culture and collaboration in the market to drive excellent patient experience through an interdisciplinary team approach.
- Collaborates with Nurse Practitioner and Care Coordinator people leaders in the market to set priorities.
- Partners with Regional Medical Director to execute on initiatives to effectively impact total cost of care and to ensure clinical interventions and campaigns are successful (i.e. optimal starts, CHF, diabetes) at managing the patient populations co-morbidities.
- Monitors clinical quality metrics and patient care delivery daily. Analyzes reports to identify operational issues or trends, or progress in meeting performance measures, and creates and deploys action plans to drive success.
- Implements strategies related to total cost of care, medical economic, and quality initiatives.
Minimum Qualifications
- Associate’s degree in Nursing, Healthcare Business, or related field.
- Active licensure as a RN, Physican Assistant, Nurse Practitioner, or other clinically related licensure.
- 7+ years combined of related education, experience, or certification.
Preferred Qualifications
- 1+ years experience managing direct reports.
- Experience managing remote teams effectively.
- Understanding of and experience in Value Based Care.
- Experience in chronic kidney and/or end stage renal disease.
- Experience hiring and interviewing

Annual Base Salary Range: $109,000.00-$136,000.00
