Position Overview: We are currently seeking a motivated Band 6 Occupational Therapist with experience in community rapid response to join our team in Epsom. In this role, you will play a crucial role in providing timely assessment and rehabilitation services to individuals in their homes, supporting their recovery and promoting their independence. Key Responsibilities:

  • Conducting rapid assessments of patients requiring rehabilitation in their homes, utilizing a holistic and client-centered approach.
  • Developing and implementing individualized treatment plans based on patients' needs and goals, focusing on enhancing their functional independence and quality of life.
  • Providing evidence-based therapeutic interventions to address a range of challenges, including mobility limitations, activities of daily living (ADL) retraining, and moving and handling risks.
  • Collaborating closely with multidisciplinary teams, including community nurses, physiotherapists, social workers, and other healthcare professionals, to ensure coordinated and integrated care delivery.
  • Providing rapid response support for individuals requiring urgent admission to rehabilitation services, facilitating timely access to appropriate care.
  • Conducting moving and handling risk assessments and providing recommendations for adaptive equipment and environmental modifications to promote safety and independence.
  • Providing education and support to patients and their families/caregivers on strategies for managing their health conditions and optimizing their recovery.
  • Maintaining accurate and up-to-date documentation of assessments, treatment plans, and progress notes in accordance with professional and regulatory standards.

Requirements:

  • Bachelor's or Master's degree in Occupational Therapy from an accredited institution.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Minimum of two years of post-qualification experience working as an Occupational Therapist, with experience in community rapid response and home-based rehabilitation.
  • Valid driver's license and access to a car for travel between patient visits.
  • Strong clinical skills and expertise in conducting assessments and delivering evidence-based interventions within community settings.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and multidisciplinary teams.
  • Commitment to delivering person-centered care and promoting recovery-oriented practices.
  • Flexibility and adaptability to meet the demands of a fast-paced and varied caseload.
