Position Overview: We are currently seeking a dedicated and experienced Band 6 Occupational Therapist with expertise in community and discharge planning to join our team in London. In this role, you will play a vital role in facilitating the transition of patients from hospital settings to community-based care, ensuring continuity of care and support for individuals as they reintegrate into their communities. This is an exciting opportunity to utilize your clinical skills and expertise to make a positive impact on the lives of patients and their families.

Key Responsibilities:

  • Conducting comprehensive assessments of patients' functional abilities and needs, with a focus on community and discharge planning.
  • Collaborating closely with multidisciplinary teams to develop and implement individualized treatment plans to support patients' transition from hospital to community settings.
  • Providing therapeutic interventions to enhance patients' independence and engagement in meaningful activities in the community.
  • Liaising with community resources, including social services, housing agencies, and community support organizations, to coordinate care and support services for patients post-discharge.
  • Developing discharge plans in collaboration with patients, families, and caregivers, ensuring that all necessary supports and services are in place to promote successful reintegration into the community.
  • Monitoring patients' progress and adjusting treatment plans as needed to optimize outcomes and promote recovery.
  • Maintaining accurate and up-to-date documentation of assessments, treatment plans, and progress notes in accordance with professional and regulatory standards.

Requirements:

  • Bachelor's or Master's degree in Occupational Therapy from an accredited institution.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Minimum of two years of post-qualification experience in a mental health or community setting, with a focus on discharge planning.
  • Strong assessment and treatment skills, with the ability to work effectively with patients with complex needs.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams and community partners.
  • Knowledge of community resources and support services available to individuals with mental health needs.
  • Commitment to delivering person-centered care and promoting recovery-oriented practices.
  • Flexibility and adaptability to meet the changing needs of patients and the organization.
