Role Overview: We are currently seeking a skilled and experienced Band 6 Physiotherapist with community and lone working experience to join our dedicated team. The successful candidate will play a crucial role in providing high-quality physiotherapy services to individuals in community settings, requiring the ability to work autonomously.
Key Responsibilities:
- Conduct comprehensive assessments of patients' physical health, functional abilities, and mobility in community settings, including private homes, care homes, and community centers.
- Develop individualized treatment plans and goals based on assessment findings, utilizing evidence-based physiotherapy interventions to optimize functional independence and quality of life.
- Deliver hands-on physiotherapy interventions, therapeutic exercises, and mobility training to improve patients' strength, flexibility, and mobility.
- Work autonomously, managing a caseload of patients independently while adhering to professional and organizational guidelines and protocols.
- Provide education and support to patients, families, and caregivers on self-management techniques, home exercise programs, and strategies to promote long-term well-being.
- Maintain accurate and up-to-date clinical documentation and communicate effectively with multidisciplinary teams, including physicians, nurses, and occupational therapists.
Requirements:
- Degree in Physiotherapy and registration with the Health and Care Professions Council (HCPC).
- Minimum of Band 6 experience working as a Physiotherapist in community settings, with experience in lone working.
- Strong clinical skills in conducting assessments, developing treatment plans, and implementing physiotherapy interventions in community settings.
- Valid driving license and access to a car for travel between patient locations.
- Excellent communication and interpersonal skills, with the ability to work autonomously and engage effectively with patients and multidisciplinary teams.
