Role Overview: We are currently seeking a dedicated and experienced Band 6 Discharge Planning Occupational Therapist to join our esteemed team. The successful candidate will play a pivotal role in facilitating safe and effective discharge planning for patients within mental health settings, ensuring seamless transitions from hospital to community-based care.
Key Responsibilities:
- Conduct comprehensive assessments of patients' functional abilities, support needs, and environmental factors to inform discharge planning and transition arrangements.
- Collaborate closely with multidisciplinary teams, including psychiatrists, nurses, social workers, and community support services, to develop personalized discharge plans that address clients' physical, cognitive, social, and emotional needs.
- Provide practical support and guidance to patients and their families throughout the discharge process, including coordination of services, procurement of equipment, and facilitation of community resources.
- Liaise with external agencies and stakeholders, including housing authorities, local authorities, and voluntary organizations, to arrange appropriate support and accommodations for patients post-discharge.
- Offer education and training to staff members on discharge planning processes, risk assessment, and strategies to promote successful community reintegration.
- Maintain accurate and up-to-date documentation of discharge plans, interventions, and outcomes in accordance with professional and regulatory standards.
Requirements:
- Degree in Occupational Therapy and registration with the Health and Care Professions Council (HCPC).
- Minimum of Band 6 experience working as an Occupational Therapist, preferably in discharge planning or a related mental health setting.
- Strong clinical skills in conducting assessments, developing discharge plans, and implementing interventions to support successful transitions from hospital to community-based care.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with patients, families, and multidisciplinary teams.
- Sound understanding of relevant legislation, policies, and best practice guidelines governing discharge planning and community mental health services.
- Commitment to delivering person-centered care and promoting recovery-oriented practice.
