Responsibilities:

  • Providing expert occupational therapy assessments and interventions for patients with Frailty and T&O conditions.
  • Developing and implementing personalized care plans to support patients' rehabilitation and recovery.
  • Covering ward duties and assisting with patient management as required.
  • Collaborating closely with the multidisciplinary team to ensure holistic patient care.

Requirements:

  • Degree in Occupational Therapy and HCPC registration.
  • Band 6 experience with a focus on Frailty and T&O.
  • Strong clinical skills in assessment, treatment, and discharge planning.
  • Excellent communication and teamwork abilities.
