Responsibilities:
- Providing expert occupational therapy assessments and interventions for patients with Frailty and T&O conditions.
- Developing and implementing personalized care plans to support patients' rehabilitation and recovery.
- Covering ward duties and assisting with patient management as required.
- Collaborating closely with the multidisciplinary team to ensure holistic patient care.
Requirements:
- Degree in Occupational Therapy and HCPC registration.
- Band 6 experience with a focus on Frailty and T&O.
- Strong clinical skills in assessment, treatment, and discharge planning.
- Excellent communication and teamwork abilities.
