Job Description: As a Band 6 Occupational Therapist specializing in General practice, you will play a crucial role in enhancing the well-being and independence of individuals through the assessment, planning, and implementation of occupational therapy interventions. You will be working in collaboration with a multidisciplinary team to provide high-quality care to patients in the Buckinghamshire region.
Responsibilities:
- Conduct thorough assessments of patients' physical, cognitive, and psychosocial needs.
- Develop and implement individualized treatment plans in collaboration with the patient and other healthcare professionals.
- Provide intervention and therapeutic activities to improve clients' functional independence.
- Monitor and evaluate the effectiveness of interventions, adjusting treatment plans as necessary.
- Maintain accurate and timely documentation of assessments, progress notes, and discharge summaries.
- Collaborate with other healthcare professionals to ensure coordinated and holistic patient care.
- Stay abreast of industry developments and best practices, integrating them into clinical practice.
Requirements:
- Bachelor's or Master's degree in Occupational Therapy.
- HCPC registration and eligibility to work in the UK.
- Proven experience as a Band 6 Occupational Therapist, with a focus on General practice.
- Strong interpersonal and communication skills.
- Ability to work effectively within a multidisciplinary team.
