We have an exciting job opportunity for a Band 6 Home First Occupational Therapist to join our NHS client’s dynamic and dedicated team based in Basildon, Essex. The post is available to start ASAP, offering full-time hours for an initial 12 week period.

The team specialize in a "Home First" approach, focusing on rehabilitating and empowering individuals to maximize their functional abilities and live fulfilling lives within their familiar home environments.

As a Band 6 Home First Occupational Therapist, you will work as a senior clinician within the CRHT West team. Occupational Therapists work as a integral part of the CRHT providing specialist bio-psycho-social interventions. You will work closely with interdisciplinary teams, including physiotherapists, nurses, social workers, and physicians, to assess patients' needs, develop personalized care plans, and implement interventions that promote independence and facilitate successful transitions.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' physical, cognitive, and emotional abilities within their home environments.
  • Collaborate with patients, their families, and the interdisciplinary team to establish goals and develop individualized treatment plans.
  • Provide evidence-based interventions and therapeutic activities to promote independence, improve functional skills, and facilitate safe daily activities.
  • Implement appropriate assistive devices and environmental modifications to enhance patients' safety and accessibility at home.
  • Offer education and training to patients, families, and caregivers on adaptive techniques, energy conservation, and strategies to manage daily activities effectively.
  • Evaluate treatment outcomes, monitor progress, and make necessary adjustments to the care plans.
  • Maintain accurate and timely documentation of assessments, interventions, and progress notes.
  • Participate in team meetings, case conferences, and continuous professional development activities to enhance your clinical skills and knowledge.

Requirements:

  • Bachelor's or Master's degree in Occupational Therapy.
  • Current registration with the relevant professional regulatory body.
  • Proven experience as an Occupational Therapist, preferably with experience in community or home-based care.
  • Sound knowledge of home adaptations, equipment prescription, and falls prevention strategies.
  • Ability to perform functional assessments and develop appropriate treatment plans.
  • Strong communication and interpersonal skills to effectively collaborate with patients, families, and the interdisciplinary team.
  • Proficiency in using relevant assessment tools and electronic documentation systems.
