Residential Licensed Practical Nurse

About the Position:
The Residential Nurse is responsible for the assistance in the medical and health related issues of clients in the Residential Program. The Residential Nurse assists with overseeing the Self-Administration of Medication Program in the Residential level of care including: supporting the MAT program, pharmacy follow up and provide daily support to providers. Assist with coverage in Detox during the absence/call out of a nurse. This staff will be the counterpart/ 2nd shift support to the Residential Nurse Lead to support Detox and Client Services where necessary
Schedule:
Tuesday - Saturday: 3:00pm - 11:30pm, off Sunday and Monday
Your Role:
- Participate in multidisciplinary treatment planning and recommend options and courses of action based on medical findings and client’s condition
- Help to identify short-term and long-range client care issues to be addressed during Residential as well as after discharge
- Attend to day-to-day health situations that may arise among clients in residential treatment setting
- Evaluate clients’ progress and symptoms, and make appropriate adjustments to medication regimens based on clients’ needs and conditions – under the guidance of licensed Medical Providers
- Assist with reordering discharge medications and discontinuing orders
- Assist with pharmacy follow up related items.
- Manage and routinely audit the processes for medication administration within the residential women’s and men’s med rooms
- Conduct medication competency training for new hire employees to entrust that all staff have passed their competency-based assessment prior to autonomously facilitating scheduled medication rooms
- Complete DPH regulatory medication training for all Client Services employees (January/July)
- Meet the immediate needs of the client by being available to provide PRN medications outside scheduled med room times
- Monitor and retrieve signed informed consents for residential and detox clients as it relates to MATs programming
- Administer MATs injections to all residential clients and ensure they receive them as ordered/schedule
- Consults with licensed providers regarding clinical (medical or psychiatric) findings and status of clients
- Refer clients to hospital for evaluation and treatment when higher level-of-care is indicated or suspected
- Refer to medical specialist for treatment or follow-up when needed
- Support Detox RN's responsibilities where necessary
Qualifications:
- Connecticut LPN licensure
Compensation:
The base rate of pay for this position is $30.00 to $35.00 per hour. Actual pay is determined based on a number of job-related factors including skills, education, training, credentials, experience, scope and complexity of role responsibilities, geographic location, performance, and working conditions.
