Aftercare Coordinator / Case Manager

Mountainside is hiring an Aftercare Coordinator to join our Continuing Care team! Schedule:
Monday through Friday 8:00 am - 4:30 pm
Your Role:
- Develops and provides client with continuing care plan/resources prior to discharge.
- Markets and promotes Mountainside’s programs and facilities by preparing and providing information, answering questions
- Accurately relay payment information necessary for admission, collect insurance information and communicate financial options
- Collaborates with client’s treatment team (clinician, family wellness) to develop a clinically appropriate aftercare plan.
- Complete all daily scheduled tasks on a day-to-day basis
- Manage a caseload of up to 15 clients.
- Provides family members, providers, and referral sources with weekly updates on status of continuing care plans and/or clients commitment to aftercare planning.
- Facilitates weekly group sessions to educate new residential clients on the continuing care process and appropriate levels of care/providers
- Maintain notes and all applicable documentation in our CRM and EMR systems
- Complete all Daily Scheduled Tasks on a day-to-day basis
- Comply with all federal, state and accreditation regulatory requirements.

Qualifications:
- Minimum High School education with 2 Years of experience in human services, addiction treatment or related field required
- Bachelor's Degree (B.S., B.A., or B.S.S.W.) from a four-year accredited college or university preferred
- Must have basic computer skills including competence in software applications such as Microsoft Office suite, Adobe and use of internet applications and in using and/or connecting wireless computer hardware, the ability to project to remote devices, etc.
- Works well independently along with the ability to work collaboratively with Clinical Treatment, Admissions, and Business Development teams at Mountainside.
- Ability to multitask/prioritize specific caseload needs.
- Ability to adapt to a changing environment.
- Ability to work both independently and collaboratively as necessary.
- Strong communication skills.
- Excellent organization and time management skills.
Compensation:
The base rate of pay for this position is $20.00 - $22.00 per hour. Actual pay is determined based on a number of job-related factors including skills, education, training, credentials, experience, scope and complexity of role responsibilities, geographic location, performance, and working conditions.
