JOB SUMMARY
The Outpatient Navigator role is responsible for assisting all patients in navigating services from Crisis/Express to all Outpatient services including Progressions Counseling, Intensive Outpatient Programming, and Progressions Psychiatric services. They are an Outpatient Ambassador and have a passion for educating all patients about Outpatient services and facilitating a warm hand-off from Crisis/Express to the appropriate Outpatient service providers.
ESSENTIAL JOB FUNCTIONS
- Acts as an Outpatient Ambassador and promotes all Outpatient services to patients and staff
- Assists patients in navigating from Crisis/Express to Outpatient Admission
- Removes all barriers for patient participation in services (i.e. transportation, tutorial for Telehealth, etc.)
- Tracks daily volume and is accountable for conversion rate of patients to Progressions
- Maintains a positive working relationship with multidisciplinary team members within Crisis/Express to encourage confidence in clinical referrals to Outpatient services.
- Distributes Outpatient collateral and educates staff on changes to Outpatient services (i.e. IOP tracks)
- Builds working relationship with patients in Crisis unit or Express to communicate about Discharge Plan and Outpatient services.
- Proficient in electronic systems including MINDq to track patient foot traffic and improve patient experience.
- Tracks reasons for Outpatient ineligibility and communicates potential solutions
- Documents outreach attempts in patient chart
- Identifies areas for process improvement and presents solutions
- Schedules Intakes with Progressions Clinical Team
- Attends Supervision and Treatment Teams as needed
- Completes other duties as assigned
POSITION QUALIFICATIONS
Minimum Qualifications:
- HSD with 3+ years experience in behavioral health field
- Level 1 Fingerprint clearance card
Preferred Qualifications:
Bachelor's degree in social work or related field
2+ years post grad experience in behavioral health field
Previous Outpatient experience
