Self-ID Questions

We recognize that people’s identities are complex. We know that these lists are still incomplete and we encourage you to use the self-describe boxes if you want to identify in a way that is not represented on these lists.

Gender and Sexual Orientation: Which of the following best describes you? If you identify as more than one gender you may select multiple options. If your gender is not listed, please use the self-describe box. *

Do you identify as transgender? (Select one) *

Do you consider yourself a member of the Lesbian, Gay, Bisexual, Transgender, Queer, Intersex, Asexual, and/or Two-Spirit, Plus (LGBTQIA2S+) community? (Select one) *
