About the Role: 
The Clinical Pharmacist is an integral member of the interdisciplinary team providing telehealth services to support Medicare patients living in rural communities.  The Clinical Pharmacist provides direct-to-patient interventions that optimize the use of medication, improve medication adherence, and promote improvement in health outcomes.  This role also provides expertise to other members of the team in the management of patient health concerns.  You will:

  • Respond to referrals from other members of the Main Street team in a timely manner
  • Offer recommendations to the Main Street provider and/or community primary care providers and specialists as appropriate, i.e. compliance with treatment guidelines for evidence-based medicine, simplify medication regimen for improved adherence, formulary alternatives, reduction of high-risk medications, evaluation of high-cost medications, etc.
  • Speak with patients in order to problem-solve difficulties with medication adherence
  • Review medication reconciliations based on identified criteria
  • Document appropriately in accordance with Main Street standards
  • Participate in case conferences for complex patients
  • Drive quality care and HEDIS measure closures
  • Willingness to obtain and maintain multiple state licenses as needed
  • Perform other duties as assigned

Requirements for This Role: 

  • Pharm D. degree from an accredited school of pharmacy
  • Unrestricted license in state(s) as requested
  • 5+ years working with high-risk adult chronic disease patients and/or seniors
  • Experience working with an interdisciplinary team
  • Experience with electronic medical records
  • Medicare and Medicaid experience preferred
  • Works and communicates well in a team environment
  • Maintains a patient-centered focus
  • Excellent communication skills
