Position: Per Diem: Licensed Practical Nurse (LPN)
Job Description:
Legacy Care Group is seeking compassionate and skilled LPNs to join our per-diem pool. This role offers flexible scheduling to accommodate your lifestyle while allowing you access to many jobs across various healthcare settings in the Boston area.
This role has a focus on community-health and providing support to individuals in underserved populations. Your experience and expertise can contribute to improving healthcare outcomes for our communities.
Schedule:
Flexible scheduling options are available including days, nights, evenings, and weekends. Contract, per-diem, part-time, full-time contracts available.
Key Responsibilities:
- Conduct thorough initial assessments, including comprehensive evaluations of social determinants of health, medical history, and mental health status.
- Provide compassionate care and emotional support to patients, employing effective communication strategies tailored to individual needs.
- Utilize nursing assessment skills to identify and address complex health issues among diverse patient populations, including those experiencing homelessness and individuals with histories of substance abuse or mental illness.
- Implement evidence-based practices and harm reduction techniques to mitigate health risks and promote overall well-being.
- Collaborate with interdisciplinary teams and community resources to facilitate access to healthcare services and support programs for underserved populations.
- Advocate for patients' rights and address systemic barriers to healthcare access and equity.
Job Requirements:
Education:
- Graduate of an accredited LPN program.
Licenses & Certificates:
- Active Massachusetts licensure as a Licensed Practical Nurse.
- Basic Life Support (BLS) certification required.
Experience:
- Minimum of 1 year of nursing experience in is preferred.
- Experience working with underserved populations.
Skills:
- Strong nursing assessment skills, particularly with diverse populations such as individuals experiencing homelessness, those with histories of substance abuse or mental illness.
- Ability to learn quickly and adapt to different healthcare environments.
- Strong communication and interpersonal skills.
- Proficiency in multiple languages is highly desirable.
