We are looking for a passionate clinician to be an advocate for the patients Heal sees in assisted living facilities.  The Patient Advocate for Assisted Living will own the entire patient experience, from educating patients and physicians on the value of Heal home visits, to scheduling patient visits and ensuring our physicians have access to all necessary clinical documentation, to managing post visit follow up, including both medical follow up and social services/community needs.

●      Educate patients and families on the value of using Heal in-home visits for primary care

○      Use brochures, one-pagers, videos, scripts, and any other means appropriate to educate patients and families on the value of Heal providers in assisted living

○      Attend assisted living facility events, such as family nights, in order to share information about the value of Heal

○      Enroll 100+ patients/month in Heal to receive assisted living care

○      Assist in developing and refining education materials based on real world experience working with patients and families

●      Support our physicians in providing a quality Heal primary care experience for patients

○      Work with our scheduler and the assisted living facility staff to ensure all patients are scheduled appropriately for their visits

○      Provide training and support to assisted living facility staff, patients, and families who will use our Heal web portal or phone app to schedule patient visits

○      Ensure physicians have access to patients’ past medical history

●      Manage post-visit follow up and ongoing patient primary care needs

○      Serve as main point of contact for patient and family requests

○      Work with Heal physicians, community organizations, other providers, and anyone else necessary to ensure patient primary care needs are met (for example, prescription refills and lab results)

●      Facilitate patient access to social and other community services

○      Become an expert on social and community services in the Bay Area

○      Build relationships with relevant care provider partners and community organizations

○      Work with patients and families to get these services set up as appropriate


●      MA, LVN, LPN, or RN certification required

●      Experience or certification in social work a plus

●      Experience working with senior care and facility-based patients




●      Strong passion for serving senior patients and improving their quality of life

●      Passion for improving the healthcare system

●      Skilled at building relationships with different stakeholders across the healthcare system (e.g. facility staff, patients, families, community organizations, physicians)

●      Adept, knowledgeable, and approachable in communicating with and educating patients and families

●      Passion for learning about the local community and services available to seniors

●      Empathetic
