Habitat Health was founded in 2023 to deliver fully integrated health and social care to older adults on Medicare or Medicaid who wish to remain living independently in their homes and communities. The company operates its model through the Program of All-Inclusive Care for the Elderly ("PACE") in collaboration with local community partners. For these older adults, Habitat Health has the opportunity to significantly improve clinical outcomes, extend lives and preserve families – whether of kin or of choice.

More than 2.2 million Americans could benefit from being enrolled in a PACE program, yet less than 5% of eligible Americans are enrolled today. Habitat Health has been designed to address challenges that have kept the PACE program subscale. Role Scope:

Our Growth team focuses on the end-to-end process of identifying, attracting, engaging with, and enrolling new participants into Habitat’s PACE program. We are looking for a Director to join our growth team to develop and execute Habitat’s strategy for building relationships with members of the community and engaging older adults to become participants with Habitat Health. As a Director of Strategic Growth & Engagement, you will:

  • Support development of / drive strategic growth initiatives across activities related to expanding the participant population
  • Develop end-to-end regional participant outreach & enrollment organization, including key strategic decisions, local enrollment & marketing team structure/staffing, and relevant policies & procedures designed around our goals, with consideration for federal and state regulations
  • Build and execute on overall community engagement strategy, developing overall organization’s approach and activities, as well as supporting execution within each region as needed
  • Support senior and clinical leadership on physician engagement strategy and activities, especially as it relates to growth initiatives (e.g., patient engagement and enrollment)
  • Collaborate across the organization, including supporting key strategic decisions and building assets related to clinical operations, participant experience and journey, and product development as they intersect with growth priorities 
  • Facilitate cross-functional & external stakeholder engagement to align on and drive progress against key initiatives
  • Monitor success, elicit feedback and input on initiatives, and identify opportunities for / drive continuous iteration and improvement

 

Qualifications: 

  • 7+ years of relevant experience, such as in health care delivery at a high-growth startup, patient growth, and community outreach
  • Demonstrated ability to be a self-starter, with ability to excel in a fast-paced, unstructured work environment, comfort operating in ambiguity, and a bias to action
  • Excellent analytical and project management skills
  • Strong interpersonal and written/oral communication skills; high EQ
  • A strong passion for our mission, with a high degree of empathy for the populations we serve
  • A bachelor's degree is required

 

Compensation: 

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, participation in the company’s equity program, and paid time off, including vacation and sick leave. The expected salary range for this position is $150,000 to $175,000. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
