Givens Affordable Communities is looking for a full-time Community Nurse to support our Asheville and Waynesville campus residents in their wellness journey. The supportive services team works together to develop and provide a program of service enriched housing with the goal of empowering residents to live well and maintain their independence. As the Community Nurse, you will provide on-site care management in our communities. The Community Nurse will plan and implement supportive wellness services that educate and empower residents, some of whom are challenged with chronic disease, nutritional problems, polypharmacy, mobility, cognitive and mental health issues, to engage in healthy self-care practices and better management of their chronic illnesses to extend their independence and improve quality of life.

 

What you'll do:

  • Conduct initial and regular resident assessments to determine the service needs of residents and to ensure that services provided address the needs of the current population
  • Work closely with the Resident Services Coordinators to identify and offer support to residents with significant medical and wellness challenges
  • Use creative educational and clinical coordination skills to successfully develop, implement, and evaluate care needs and outcomes as an empathetic care advocate
  • Support the campus emergency pendant response system protocol. Provide the appropriate follow-up assessment after an event to check on the resident
  • Provide a clinical perspective in care, early identification of potential risk factors, healthcare or self-care needs, level of care transitions, and educational opportunities to help individuals improve health
  • Work closely with Resident Services Coordinators to develop provide the supportive services required to put ‘whole person health’ solutions into place that meet the resident’s specific needs
  • Address physical, psychosocial, emotional, educational, spiritual, or transitional care needs of residents
  • Assist LifeMinistries or Supportive Services Manager with health and wellness outreach events, educational programs, and planning
  • Work with team members and residents to implement infection control measures and guidelines for the health and safety of the communities
  • Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor

What you'll need:

  • Bachelors prepared nurse 
  • Minimum 2 years of care coordination/case management experience 
  • Experience with geriatric population 
  • Background in health coaching, one-on-one and group wellness education using teach-back and motivational interviewing techniques
  • Educational/clinical preceptor or nursing supervisory experience preferred 

Compensation is $83k - $90k per year, based on background and experience, and includes our comprehensive benefits package.
