Givens Choice is looking for a full-time Care Navigator to join our growing team! This position’s primary function is to build and maintain a professional relationship with Members, their support networks, physicians, service providers and all other entities involved in the Member's care. This team member will provide an individualized plan of care for each Member of the program using effective and efficient geriatric care management techniques. 

What you'll do:

  • Implement and manage current operational processes and procedures in accordance with the standards & procedures set out by the Executive Director and Lead Care Navigator
  • Meet personally with prospective clients to answer questions about the program and perform assessment components of the membership application process
  • Develop and implement Member Wellness Plans and Member Support Plans using established procedures
  • Participate in the on-call rotation
  • Maintain up-to-date files on all resources available in the community for seniors
  • At the request of the Executive Director and Lead Care Navigator, conduct marketing meetings with prospective Members to educate prospective members about Givens Choice
  • Compile a monthly report including, but not limited to, visits, amounts of service provided by category, QA results, etc.
  • Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor

What you'll need:

  • Social Work, Nursing, or Health Services related degree with at least 5 years’ experience working with older adults in healthcare environment. Care management experience strongly preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
  • Excellent written and verbal communication skills with the ability to engage in public/Program activities to promote the mission of the organization and the Givens Choice program
  • Complies with accepted professional standards and practices
  • Demonstrated ability to be self-directed with little supervision and self-motivated in fulfilling work tasks, obligations, and expectations

Compensation will be $26-$27 per hour based on experience, and includes our comprehensive benefits package.
