About the role:

FORM is looking to support its growth in the employer benefit space with the expansion of its Care Advisor team. The Care Advisor will be a critical contributor to realizing FORM’s ambitious growth goals by engaging with prospective patients to understand their needs, educate about FORM and help them evaluate if FORM is appropriate for them. The Care Advisor is the first point of contact with FORM and therefore responsible for helping patients understand the FORM value proposition and details of the FORM program along with answering questions. The Care Advisor is responsible for nurturing inbound leads through multiple channels (email, phone, text) to convert appropriate leads into FORM patients. You’ll be successful in this role if you bring empathy, high-energy, adaptability, and strong verbal and written communication skills. As a member of an early stage team, you’ll get the opportunity to shape the tools and processes for best-in-class patient acquisition, and you’ll partner with Marketing, Product, Engineering, and Patient Care to implement your insights and recommendations.

Where you’ll deliver impact: 

The Care Advisor team supports prospective patients nationwide. As such, Care Advisor candidates must be able to work Monday through Friday from 12 - 8pm ET / 9am - 5pm PT.

Key responsibilities include:

  • Drive new patient growth by converting prospective patient leads who are appropriate for the FORM program. You will understand prospective patients’ unique needs, effectively communicate the FORM value proposition, and answer prospective patient questions. You’ll meet (and exceed) monthly, quarterly, and annual new patients growth targets.
  • Nurture inbound prospective leads by managing all communication with prospective patients inclusive of initial outreach and ongoing communication. Communication will include email, text, and phone calls.
  • Develop new, creative strategies to effectively engage and convert prospective patients.
  • Partner cross-functionally to measure effectiveness of our patient development efforts and manage ongoing tracking and reporting of results.
  • Provide ongoing prospective patient feedback to teams across FORM. Report and highlight prospective patient needs to ensure we are prioritizing the most impactful communications, products, and services.

You’ll be successful in this role because you have:

  • 2+ years of hands-on experience in sales, customer service, or another customer-facing role.
  • Experience in healthcare, ideally working 1:1 with patients to assist them in accessing care or making treatment decisions.
  • Passion for health care and improving the life of patients. Excitement to get involved in the obesity care category and join FORM on its mission to expand access to high quality clinical obesity care.
  • Genuine empathy and passion for engaging with patients and strong interpersonal skills.
  • Comfort working in a fast-paced, ever changing environment.
  • Incredible verbal and written communication skills with strong attention to detail.
  • Experience effectively working cross-functionally with internal teams and partners.
  • High energy and motivation to be part of a startup team.
  • Commitment to continuous learning and improvement.
  • BA or BS required
