JOB TITLE: Medical Records Assistant

DEPARTMENT: Medical Records

REPORTS TO:  Medical Records Manager

FLSA STATUS: Non-Exempt

 

POSITION SUMMARY

Serves as clinical support personnel, under the direction of a registered nurse (RN), in accordance with established Federal and State regulations, accreditation standards and St Louis Eye Surgery and Laser Center (SLESLC) policies and procedures.  Maintains a clean, orderly and safe environment.  Maintains medical records, both in hardcopy and electronically. Maintains post-operative patient follow-up data.  Maintains post operative notes.  Identifies and reports to the supervisor inventory/ supply needs.

  • Ability to handle stress associated with prioritizing multiple tasks

  • Ability to recognize chart deficiencies and the ability to implement a plan of corrective action
  • Follow up with deficient individuals to have them clarify their charting
  • Scans and attaches patient medical records in accordance with SLESLC’s standards
  • Closes charts with accuracy and verifies completeness
  • Transcribes physician post-operative follow-up logs, organizes logs and checks follow-up logs for completeness
  • Ability to create patient charts with accuracy having a basic knowledge of physician preferences
  • Maintains the femtosecond file attachment and thumb drive maintenance of those files
  • Efficiently orders Airgas when instructed to do so by nurse manager
  • Ability to maintain and shred patient paper charts
  • Maintains effective communication with staff and physicians
  • Work directly with physicians and their offices in obtaining and attaching operative notes to patient electronic medical record efficiently and accurately

  • Participates in continuing education/in-service training as needed, staff meetings, and the QAPI/Infection Control programs
  • Ability to work independently and within a group
  • Follows physician / nursing orders and directions from management
  • Actively participates in the development of a healthy work environment
  • Assists in training new staff when needed

 

Attendance

  • Arrives to work area on time and ready to work. Minimal absenteeism

EDUCATION/ REQUIREMENTS

  • High School diploma or GED
  • Basic Cardiac Life Support (BCLS) required
  • Must be able to provide proof of their identity and their right to work in the United States

LANGUAGE SKILLS/COMPUTER SKILLS

  • To perform this job successfully, an individual should have knowledge of:
    • EMR Database software: Amkai Office, Amkai Charts
    • Microsoft Word and Excel.
    • PC (Windows OS)
    • Office equipment: Printer, copier, scanner, telephone, and fax

 

PHYSICAL DEMANDS

The employee must be able to:

  • Walk, stand and sit for prolonged periods of time
  • Occasionally be able to lift and/or move up to 25 pounds
  • Occasionally be able to push up to an excess of 200 pounds
  • Occasionally be able to pull/ lift patients with the assistance of other personnel and or assistive devices, up to an excess of 400-500 pounds
  • Occasionally be able to bend, stoop, kneel and climb

 

ACCIDENT OR HEALTH HAZARDS

  • Regular exposure to blood and body substances
  • Exposure to needle sticks possible
  • Bloodborne pathogen risk classification category #2

 

WORKING CONDITIONS

  • Works in well-lit and ventilated surroundings including pre-operative/ PACU areas, nurse's stations, open office, lounge, etc.
  • May occasionally be exposed to outside environment

 

Education and/or Experience

High school diploma or general education degree (GED); or one-year related experience and/or training; or equivalent combination of education and experience
