As a social worker/mental health counselor team leader, you will provide administrative and clinical oversight for a team of social workers and counselors working with patients in-person and telephonically. You will also provide direct patient care and liaise with primary care providers.
Are engaging and personable
Are strong communicators: you can communicate clearly with and quickly build rapport with patients and your team in person and over the phone
Are experienced with the tools of outcome- oriented care (specifically use of PHQ9 and GAD7)
Are experienced in motivational interviewing, behavioral activation or problem solving treatment and have experience with population health, registry management and quality improvement
Are super organized and a strong multi-tasker
Ideally have experience with Collaborative Care, or with practicing in primary care or integrated care settings
Are not intimidated by learning new technology (EMR’s, G-suite, care management platforms) and teaching it to others
Have an active AZ license (social worker - LCSW preferred, LMSW also acceptable, or LPC, LMFT, LMHC also acceptable)
Be based in the Phoenix (or Tucson) area and able to travel to clinics around the city pending COVID restrictions being eased (when you are not on-site, you will be able to work from home)
