Weekly outpatient therapy isn't always enough, and a trip to the ER isn't the only answer. Patients and their families rely on Compass Health Center when in crisis – every day, we help people overcome depression, anxiety, suicidality, obsessions/compulsions, trauma, chronic pain, and other obstacles in order for our patients to live freely.
About This Role
We are seeking a Discharge Planner for our Over 18 Programs, with experience working with Young Adults, Adults, and their families. You will be responsible for facilitating aftercare planning with patients and families as well as supporting Primary Clinical therapists with administrative tasks, as needed. If you are passionate about working with clinical teams and families to communicate, develop, and carry out appropriate aftercare referrals and compassionate care, you would be an ideal addition to our team.
Our centrally located Chicago office is nestled in the vibrant neighborhood of North Center, not to mention the social events hosted on our fully furnished rooftop!
What You'll Do:
- Support patients/loved ones in aftercare planning including therapy, psychiatry, and group referrals
- Collaborate with primary clinical therapists to understand patient needs and develop supportive plans
- Manages referral lists, ensure accurate information regarding available appointments and develop relationships with referral sources
- Problem solve difficult discharge issues including limited insurance pools and specific discharge recommendations/requests
- Communicate with treatment teams about progress towards individual patients’ needs
- Facilitate communication among important stakeholders supporting patients inside and outside the organization
- Gather feedback and collects data about discharge processes; respond to this feedback by making improvements and necessary changes
Who You Are:
- Bachelor's Degree
- Experience working with adults
- Excellent time management
- Ability to juggle multiple demands simultaneously in a fast paced and changing environment
- Case management experience preferred
