***This position is covered by the SSG Fox Suicide Prevention Grant Program, which runs from October 1, 2024 - Sept 30, 2025 with an anticipated renewal annually through Sept 30, 2027.***
Job Summary
Clear Path for Veterans, Inc. (CPV or Clear Path), is Upstate New York’s Veteran Resource Center serving as a hub of information, programs and services for veterans and their families. The SSG Fox Family Program Coordinator is responsible for assisting military families with a loved one experiencing mental and/or behavioral health concerns, including suicidal ideation, by connecting them with programs and services that will reduce the individual's risk of suicide. This position will also provide emotional support and referrals to appropriate agencies for Veteran families who are going through a crisis or problematic situation. The Family Program Coordinator functions as a positive role model for all Veteran families through displaying a positive, encouraging approach to problem-solving and offering appropriate solutions as needed.
Primary Functions
- Respond promptly and with compassion and understanding to inquiries from Veteran
- Answer Veteran family/client questions, including researching information to provide complete and accurate responses when necessary.
- Ability to identify areas of need among a Veteran’s family members and/or the Veteran and make appropriate referrals to other available Clear Path programs and services or external resources as needed that reduce the risk of suicide.
- Meet with SSG Fox SPGP Team to review open cases and provide support when appropriate and
- Conduct targeted outreach and engagement activities to identify eligible Veterans and their families to ensure they are connected to the VA for services, as well as Clear Path's other supportive services and/or other community
- Develop and disseminate reports as necessary including, but not limited to, outreach events, the activities CPV provides to the community, meetings with community partners, detailed categories of Veterans and family members served.
- Monitor programs’ compliance with grant requirements and any other contractual agreements to ensure adherence and that all deadlines are met.
- Utilize Salesforce platform to enter program data and generate
- Evaluate Family support service effectiveness and efficiency through monitoring all caseloads to ensure Veteran family needs are holistically met.
- Search out and identify potential resources within assigned territories, including, but not be limited to community, state, federal, health, and social resources.
- Represent, or assist in representing Clear Path for Veterans at a variety of community events and other
- Identify and form relationships with Veteran family members encountered at those
- Prepare and facilitate a variety of presentations about Clear Path’s family support service and Clear Path
- Attend and actively participate in departmental and organizational
- Promote teamwork by communicating regularly and effectively with the SSG Fox SPGP
- Manage submission of credit card receipts to the finance
- Design and manage family events at Clear Path for Veterans campus, virtually, and off-site when
- Assist with social engagement and integrative medicine activities as
- Complete other related duties as
Education and Experience Requirements
- Must be an immediate family member of a Veteran or military
- Associates degree required; bachelor’s degree preferred in a related
- Must have a high school diploma or GED
- Must have a minimum of one (1) years’ work experience in a Veteran focused/related Verifiable volunteer experience will be considered.

Job-related Physical Abilities Needed in Order to Perform the Job in a Satisfactory Manner
Must be able to lift a maximum of thirty-five (35) pounds. May sit or stand for long periods of time. Manual dexterity sufficient for constant use of computer keyboard and other office equipment. Vision sufficient for frequent use of computer monitor; to read and develop paperwork.
