Under the direct supervision of the Enhanced Care Management Director, the Lead Care Manager provides high quality case management to eligible Chaparral Medical Group patients with complex medical and social needs. They will work with leadership, providers and a multi-disciplinary care to determine the needs of high acuity, vulnerable patients and provide basic housing assistance, patient tailored intensive case-management, developing a care/service plan; provide linkages to medical, psychiatric, social, educational and other services as needed. 

In this position the Lead Care Manager will:

  • Outreach to eligible health plan members to enroll them into Enhanced Care Management per protocols.
  • Conduct a comprehensive assessment with each client including demographic information, psychosocial history, and SDoH status.
  • Develop and implement an individualized care plan with each client that is updated over time.
  • Provide case management services to medically and socially complex high risk individuals.
  • Act as client advocate, and provide intervention and support to clients when appropriate.
  • Provide appropriate information regarding referrals, ensuring linkage.
  • Advocate with primary care providers, including Chaparral’s Street Medicine Teams, to ensure access to quality treatment and/or care.
  • Perform administrative duties, such as assist in development of reports, build and maintain a contact network of agencies and local community members, participate in the Quality Management plan, and perform other duties as assigned.
  • Serving as the primary point of contact for the Member, Member’s family, Authorized Representative (AR), caregiver, other authorized support person(s) as appropriate, and the multidisciplinary care team providing care to the Member.
  • Work closely with and travel regularly with the Street Medicine Team to coordinate care for members

Requirements

  • High School Diploma or equivalent
  • Knowledge of community resources, health education methods, advocacy, harm reduction and general clinical knowledge
  • Ability to create and maintain boundaries with clients
  • Ability to complete required program documentation in timely manner
  • Skills in managing multiple deadlines and demands, and the ability to follow through on assignments
  • Ability to work independently and as part of a group
  • Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality
  • Ability to work effectively with people of diverse cultural and educational backgrounds
  • Ability to maintain cooperative working relationships with staff members
  • Ability to communicate effectively, including articulating one’s own relevant personal experiences
  • Travel 50-75% - Must have a valid driver’s license, automobile insurance and reliable transportation

Preferred

  • Degree in Social Work or similar field
  • Bilingual in English and Spanish strongly preferred
  • 2+ years of experience with and comfort working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).

Compensation: $22/hr to $28/hr. Variable based on qualifications, experience, and other interview factors.

Physical Demands: Most work will be in the field working with CalAIM populations (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs). Will likely require substantial walking, standing, sitting in a car, and driving in various weather conditions.
