The Registered Nurse (RN) will be responsible for providing street medicine services to CalAIM populations in Bakersfield, CA.

Under supervision of a physician, the RN will functions as a service provider, educator, coordinator, advocate and facilitator of health and health services for patients. These patients will include people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, and adults with complex medical needs.

The RN will work collaboratively with the Street Medicine Team to improve the quality of patient care through ongoing communication and collaboration with the multidisciplinary care team, including case management for chronically ill patients.

Job Duties

Patient Care:

  • Utilize nursing process and provide professional nursing care in a street setting, include assessing medical needs and compiling a care plan, addressing acute needs, chronic conditions, and preventive health maintenance
  • Provide basic medical case management to patients, including on an as needed basis: communicating with patients or families, helping execute a care plan, reconciling patient medications, providing interventions as appropriate, and helping with clinical follow up to ensure patient self-management or self-care goals are being addressed
  • Work closely with the partner organization to provide whole person care for clients on the streets, practicing harm reduction approaches, considering creative and flexible solutions, and partnering with the clients to take steps forward with their health
  • Dispense medications, according to approved standardized procedures, including nebulizer treatments and giving medication injections and vaccines
  • Administer medications in street medicine setting when appropriate
  • Document all patient interactions as deemed appropriate in the patient chart in the EMR, taking care to chart in an informative, accurate and descriptive manner that reflects the patient care provided and keeping accurate records on intervention and patient education
  • Participate in patient follow-up activities as directed by providers for normal and abnormal laboratory, DI or specialty consult results
  • Provide patient education and information about chronic care, disease prevention and the promotion of health maintenance
  • Skill in wound care is strongly preferred

Management / Communication:

  • Ability to work autonomously, taking initiative and problem solving within the scope of an RN
  • Ability to determine when to function independently and when to request physician intervention
  • Ability to manage a high volume of patients in a short span of time, while dealing with unpredictable issues
  • Prioritize daily workload to ensure that all responsibilities are carried out in a timely manner
  • Function as a member of the care team by participating in care team meetings and communicating with all members of the team to facilitate patient care
  • Utilize appropriate methods for interacting with and collaborating sensitively, effectively and professionally with persons from diverse backgrounds
  • Provide crisis intervention when required to promote a safe and peaceful setting and keep patients and staff safe

Other:

  • Ability to operate a computer to obtain lab results, log clinic visits, and input/retrieve data
  • Ability to work in a dynamic, outdoors environment
  • Maintenance of reliable transportation
  • Knowledge of regarding nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to community health
  • Ability to travel 50-75% - Must have a valid driver’s license, automobile insurance and reliable transportation
  • Commitment to consistently support compliance by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, and accreditation and licenser requirements (if applicable)

Requirements

License, Certification, Registration:

  • Diploma from an accredited nursing school
  • Current, unencumbered, active California license to practice as an RN
  • CPR certificate

Preferred:

  • Bilingual in English and Spanish strongly preferred
  • 1+ years of experience with and comfort working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).

 

Compensation: $45-60/hr. Variable based on qualifications, experience, and other interview factors.

Physical Demands: Most work will be in the field working with CalAIM populations (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs). Will likely require substantial walking, standing, sitting in a car, and driving in various weather conditions.
