About Cayaba Care

Cayaba Care is a building support service to reimagine maternity care and create a new standard for healthcare, especially for Black and Brown communities. The US has some of the least favorable outcomes among developed nations and is the only country not to guarantee access to provider home visits and these poor maternal-fetal outcomes disproportionately impact women of color and underserved populations across major US cities. Our team is focused on building technology-enabled solutions to deliver holistic, home-based clinical and non-clinical maternity services that are supplemental to a patient’s primary obstetric care to reduce barriers and improve patient outcomes.

Our Values:

Healthcare is Human: We partner with the communities we serve to address the unique needs of our patients and support whole-person care.

Pursue Excellence: We earn & build trust with our patients, partners, & each other by striving for excellence without compromising our standards.

Uplift: We elevate and center underrepresented voices.

Embrace Innovation: We are creative & resourceful in getting things done.

Together, not alone: Moving maternal healthcare forward is a shared responsibility that we all care deeply about. To learn more about Cayaba Care, please visit www.cayabacare.com.

Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
