Cadre Hospice seeks to expand access to the hospice benefit, especially among underserved populations. In addition to traditional go-to-market channels, Cadre plans to partner with at-risk medical groups to integrate upstream and offer a comprehensive palliative care program to patients nearing end of life. We are seeking an experienced and compassionate Hospice Registered Nurse (RN) Case Manager to join our interdisciplinary team and provide comprehensive care coordination and support to hospice patients and their families. The RN Case Manager will be responsible for assessing patient needs, developing individualized care plans, coordinating care services, and providing emotional support throughout the end-of-life journey.
Key Responsibilities:
- Clinical Assessment and Care Planning:
- Conduct thorough physical assessments of hospice patients to evaluate their medical condition, symptoms, and psychosocial needs.
- Collaborate with patients, families, and interdisciplinary team members to develop personalized care plans that address the unique needs and preferences of each patient.
- Monitor patient responses to treatments and interventions, adjusting care plans as necessary to optimize comfort and quality of life.
- Symptom Management and Pain Control:
- Assess and manage pain and other symptoms using evidence-based pharmacological and non-pharmacological interventions.
- Administer medications safely and accurately, including opioids and other pain management medications, while monitoring for adverse effects.
- Provide education and support to patients and families on symptom management techniques and medication management.
- Emotional Support and Counseling:
- Provide emotional support and counseling to patients and families facing end-of-life issues, including grief, loss, and anticipatory mourning.
- Foster open communication and trust with patients and families, listening to their concerns and addressing their emotional and psychosocial needs with empathy and compassion.
- Assist patients and families in coping with the challenges of terminal illness, facilitating discussions about advance care planning and goals of care.
- Care Coordination and Collaboration:
- Coordinate interdisciplinary care services for hospice patients, including nursing, social work, chaplaincy, and volunteer support.
- Facilitate communication and collaboration among team members to ensure coordinated and holistic care delivery.
- Coordinate care transitions and discharge planning, including referrals to other healthcare providers and community resources as needed.
- Participates in On-call duties as defined by the On-call Policy
- Documentation and Compliance:
- Document patient assessments, care plans, interventions, and outcomes accurately and timely in electronic health record (EHR) systems, adhering to regulatory requirements and organizational policies.
- Ensure compliance with Medicare Conditions of Participation, state licensure regulations, and accreditation standards related to hospice care.
- Participate in quality assurance and performance improvement activities to enhance patient care and organizational effectiveness.
Qualifications:
- Current Registered Nurse (RN) license in state of practice
- Minimum of one year of nursing experience; hospice, palliative care, or a related field preferred.
- Strong clinical assessment and care planning skills, with the ability to develop individualized care plans based on patient needs and goals.
- Excellent communication, interpersonal, and counseling skills, with the ability to build rapport and establish trust with patients and families.
- Knowledge of hospice philosophy, principles of palliative care, and end-of-life issues, with a commitment to providing compassionate and dignified care.
- Ability to work effectively in a team-based environment, collaborating with interdisciplinary team members to achieve optimal patient outcomes.
- Proficiency in electronic health record (EHR) systems and documentation practices.
