By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.
We are currently seeking a Registered Nurse to serve as a Home Health Nurse Case Manager in Marin County andcoverage to San Francisco as needed. The home health nurse will monitor patients’ vitals, provide medication management, ensure patients are comfortable, and coordinate with other clinicians to maximize patient rehabilitation.
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):
Takes patient’s pulse, listens to heartbeat, checks temperature, and checks blood pressure.
Makes note of other vital statistics and keeps a record of these details in the patient chart.
Asks patients about pain levels and provides treatment accordingly.
Ensures that patients are fed, bathed, and well cared for. Reports any patient safety concerns right away to the proper authority and supervisor. Request MSW referral when appropriate and coordinate with MSW to ensure patient safety.
Education/Experience:
BSN degree preferred.
Minimum of 1 year experience as a Registered Nurse
Certificates, Licenses, Registrations:
Current license as a Registered Nurse in the State of California.
Current license as a Public Health Nurse in the State of California is highly desirable.
Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
Pay range: $58.00 - $72.00 hourly
