About the Role
As the Aftercare Case Manager/PAL Training Facilitator, you will impact lives through case management services for young adults ages 18-21 referred to the program from the Texas Department of Family and Protective Services. Case Management and Aftercare Services will include experiential independent living training, practical application of six identified core elements, crisis intervention, discharge planning, guidance and support to the target population. This role is critical in shining hope in the lives of these young adults.
What You’ll Do
As a member of our team you will have an impact on our clients through accomplishment of the following responsibilities:
- Provide case management services designed to enhance youth’s ability to successfully transition to adulthood and life on their own.
- Provide group training designed to teach and/or enhance independent living skills by utilizing the PAL curriculum and teaching the six core elements.
- Coordinate the pre- and post- independent living skills assessment on all youth receiving services.
- Seek community resources to enhance PAL Training services.
- Administer and review results of the post independent living assessment to referred youth.
- Provide individual independent living training to youth that score below the mastery on the essential core elements.
- Keep in constant communication with DFPS PAL staff.
- Develop and maintain resource guide that is user friendly for youth and caseworkers.
- Responsible for the coordination of check writing services through intensive case management and after care room and board.
- Assist youth in obtaining supplies needed to set up household and with transportation needs.
- Provide supportive counseling information and any other special needs identified to assist youth with emancipation.
- Work with other agencies to coordinate after care room and board services and resources available to target youth population.
- Visit youth at least twice a month while in foster care and once a month after emancipating from foster care system.
- Promote community awareness regarding the needs of youth as well as developing needed resources.
- Provide administrative tasks including: entering data for program participants into the ER system; audit charts and documentation required for program licensing; submitting documentation on a monthly basis to DFPS; receive and process approval forms for youth attending training; and creating and submitting program reports.
- Travel as needed to provide services to eligible families and help develop programs to enhance the delivery of services to individuals.
- Work a flexible schedule which may include some evenings and weekends.
What You’ll Bring to the Team
To be successful in this role and a great addition to our team we need you to come with the following:
- A Bachelor's Degree in Social Work or a related field of study plus a minimum of two years of full-time paid experience working with youth;
- Ability to observe and make accurate assessments of situations when working with youth;
- Ability to use professional discernment skills and maintain confidentiality;
- Ability to use analytical thinking to solve problems and present viable solutions;
- Ability to effectively and professionally facilitate meetings and workshops to clearly deliver information to individuals and others;
- Age 21 with a valid driver's license and a clean driving record so you can be insured on the Buckner insurance and provide transportation as needed. Requires personal transportation to provide services throughout the local community;
- Ability to work evenings and weekends and be on-call as needed;
- Ability to work with clients from all racial, social-economic and religious backgrounds

- Excellent communication skills and the ability to use tact and courtesy in interactions with coworkers, individuals and others.
Preferred Qualifications
In addition to the amazing qualifications and experiences listed above, if we had our say, we would also look for:
- Master’s Degree in Social Work or other related field plus a minimum of 1 year of full time paid experience working with youth;
- Prior related experience using community resources to serve youth in transition;
- Knowledge of family systems, crisis intervention, and resolution;
