Part Time Health Navigator

The part time health navigator monitors alerts and screens participants for medical or social concerns. The health navigator schedules and participates in audio-visual wellness calls with participants who require remote patient monitoring in the home. They will report abnormal values or other participants concerns to the Registered nurse. The health navigator acts as a liaison for the participant to ensure that all needs are met and screens for any social or medical determinants. They will work with members of the clinical care team to enhance better health and take the initiative to ensure clinical care and related support services are made available to participants. They will think critically and creatively about what is needed to enhance health improvement while assisting with the low-level tech support and troubleshooting for participants.

Responsibilities

  • Monitor all alerts.
  • Answer, direct, and follow up on incoming calls.
  • Review and maintain daily wellness call schedule.
  • Receive and perform wellness calls and document all information obtained on the call in the appropriate charting systems.
  • Support participants physical and emotional wellness with a holistic health approach.
  • Addresses social determinants of health by supporting participants through loneliness, anxiety, and depression.
  • Communicate concerns, changes in condition and all medical complaints to the Registered Nurse.
  • Troubleshoot issues with technology and escalate technical difficulties that cannot be resolved.
  • Other duties as assigned.

Requirements

  • High school diploma or GED required.
  • Strong computer skills and ability to learn and use new technology.
  • Empathy, compassion, and patience.
  • Superior listening, verbal, and written communication skills.
  • High level of professionalism and ability to work with a team remotely.
  • Strong problem-solving abilities with attention to detail.
  • Ability to handle stressful situation appropriately.
  • Adaptability, patience, and a positive attitude.
  • Must be able to work from home with a HIPPA compliant environment.
  • Must be available to work 11:30 AM-5:00 PM EST M-F along with some holidays and weekends as needed.

Compensation will range from $18/hr - $20/hr depending upon experience.
