Self-ID Questions

We recognize that people’s identities are complex. The Equal Opportunity Employment Office options do not usually capture how people identify.  To work toward a better solution, we are providing these Self-ID options in the Voluntary Demographics section below. You may choose: "I prefer not to answer" any or all of the self-ID questions.

At MHCD we embrace diversity and inclusion, and your self-ID will only be used for operational purposes, in aggregate form and has no impact on your application.

We know that these lists are still incomplete and we encourage you to use the self-describe boxes if you want to identify in a way that is not represented on these lists.

Gender and Sexual Orientation: Which of the following best describes you? If you identify as more than one gender you may select multiple options. If your gender is not listed, please use the self-describe box. *

Do you identify as transgender? (Select one) *

Do you consider yourself a member of the Lesbian, Gay, Bisexual, Transgender, Queer, Intersex, Asexual, and/or Two-Spirit, Plus (LGBTQIA2S+) community? (Select one) *

Race and Ethnicity: Which of the following best describes you? Select as many options as apply or use the self-describe box if needed. *

Describe your disability status? This could include physical, auditory, visual, cognitive and developmental disabilities as well as mental health disorders, learning disabilities, health conditions, and others that may be considered “invisible" (Select one) *
